Benefits end; agency thinks woman dead

TORRANCE Patricia Flack, 65, discovered the problem two months ago when she received a letter from Medi-Cal, saying those benefits had also been cancelled.

"Well, when I first saw it, I didn't think it looked very official, so I was going to ignore it, but I tend to need to read everything. And so I read through it and I couldn't believe it was saying that it had received notice of my death, so I was kind of shocked," Flack said.

Social Security eventually wired Flack a major portion of her benefits, telling her the rest is on its way.

A Social Security spokesman says the agency can't tell for sure how they received the erroneous death report.
