The response to the nuclear crisis in Japan last March was a failure, according to a new report by the Japanese government.

It says the risk of a reactor meltdown from a powerful earthquake was underestimated.

After the tsunami struck, there was a false assumption that emergency cooling systems were working.

Employees were confused and failed to communicate properly.

And the report says there were unnecessary delays in evacuating nearby towns threatened with radiation.
