James Hooker, 41, is charged with sexually abusing a different 17-year-old girl in 1998.
A judge ordered him to surrender his passport after police said they found a notebook detailing his plans to leave the country. Hooker was arrested on Friday.
In February, Hooker left his wife and three children for 18-year-old student Jordan Powers, who dropped out of school to be with him. Powers said the relationship is now over.
