The incident occurred around 8 a.m. Saturday morning at a home at the intersection of Sapphire Canyon Road and Highland Valley Road.
The victims were live-in patients at the facility and pronounced dead at the scene. Their cause of death has not been released, but foul play is suspected.
The sheriff's department also said the victims were not shot. A coroner will determine the cause of death.
A live-in caretaker, identified as Jianchun Li, is in custody for the murders.
Neighbors say there is usually eight elderly people who live at the facility.
Witnesses did not hear any gunfire, but were startled by the heavy police presence.
"Care workers were coming out, you know, hands up. Everyone is being escorted out and the paramedics were starting to bring out patients. So, one patient comes out in a stretcher, another one in a stretcher. They loaded her up in a paramedics unit," said Ivonne Trypucko, the manager of Deane Homes Swim Club, which is across the street from the facility.
Officials say there are no community safety concerns at this time.
